We work as strategic partners with PHNs to realise community-led health outcomes. Our co-design approach ensures that local health solutions are actually fit-for-purpose and responsive to the people who need them most.
We can work together to:
- Build pathways that connect mental health, aged care, and alcohol/other drug services into a seamless experience
- Develop models that leverage lived experience to provide support that clinical professionals often can’t
- Collaboratively develop service models and commission policies and practices to meet diverse needs.
- Support the take-up of peer-to-peer models to support transitions between services and activate latent community capability
- Build capabilities within PHNs and local systems to embrace co-production at every stage of the commissioning cycle.
- Utilise deep listening and qualitative research to inform regional health priorities
Our experience
We’ve partnered with PHNs across Australia to redesign critical health pathways. Recent work includes:
- Brisbane South PHN: Redesigning recommendations to better connect mental health and alcohol and other drug (AOD) systems.
- Western Victoria PHN: Leading co-design and capability building for forensic mental health services.
- Peninsula Health (Victoria): Co-designing post-suicidal support systems (HOPE) to align with statewide mental health reform.
Featured case study:
Connecting the Brisbane South Health System
Context: PHNs often struggle to bridge the gap between mental health and AOD services, leading to fragmented care.
Scale: An intensive 12-month co-design project involving three major health providers and over 50 people with lived experience.
The work: Facilitated deep-listening sessions and collaborative workshops to identify systemic “sticking points” in the regional referral process.
The outcome: Delivered recommendations for a more connected system, currently being implemented to ensure all community members can seek support.
